I cant see anything obvious from the code you have posted. Can you add a screen shot of the YSOD? Is there a stack trace or anything which would help people debug this with you?

Do you have any server side code the formview events? I added the stack trace information and I do not have any server side code that relates to the formview other than what I listed above about getting some information from the formview.
